AdmissionFormView class
Formulario para crear un nuevo ingreso hospitalario.
Campos obligatorios: paciente (Autocomplete con debounce), servicio (Autocomplete con debounce), diagnóstico principal e historial médico. Campos opcionales: alergias, tratamiento crónico e índice de Barthel basal (con calculadora integrada BarthelCalculatorDialog). Al enviarse muestra un diálogo de éxito/error y resetea el formulario.
